INFORMATION ABOUT THE ONTARIO STUDENT RECORD (OSR)

This webpage applies to requests for a copy of the Ontario Student Record (OSR) only.  District School Board of Niagara (DSBN) Legal Services processes requests for the complete Ontario Student Record (OSR) received from former students (or their authorized legal representatives) who have previously attended a DSBN Secondary School or Adult Learning Centre (or schools within the former Lincoln County and Niagara South School Boards) as well as requests received from students (or their parents or their authorized legal representatives) who are enrolled in a District School Board of Niagara Elementary School, Secondary School, or Adult Education Centre.

IMPORTANT NOTES BEFORE YOU PROCEED:

  1. The Ontario Student Record (OSR) is confidential. Access to the OSR will require proof of identification and payment of a fee.
    1. If the student is 18 years of age or over, the student or a parent or guardian may request and receive an OSR with written authorization from the student, including proof of the student’s identification. Students must complete: Consent to the Disclosure, Transmittal or Examination of School Record Form (to be used by student age 18 and over).   
    2. If the student is under 18 years of age, a parent or guardian may request and receive an OSR with written authorization from the parent or guardian, including proof of the parent’s or guardian’s identification.  Parents or guardians must complete: Request for the Disclosure, Transmittal or Examination of School Record Form (to be used by parent of a student under age 18).
  2. In compliance with the Municipal Freedom of Information and Protection of Privacy Act (MFIPPA), the DSBN Legal Services is not permitted to release any student information over the phone.  

ORDERING AN ONTARIO STUDENT RECORD (OSR) (excludes transcripts and diplomas)

  • Please contact Legal Services, District School Board of Niagara, 191 Carlton Street, St. Catharines, ON L2R 7P4 Phone: 905-641-2929 ext. 54144 Fax: 905-641-5060 if you require a copy of your Ontario Student Record. Please note that during the summer months, from July to August, requests for copies of OSR documents can still be made to DSBN Legal Services, however, requests will not be completed until September when school resumes.
  • You may order secondary school transcripts, replacement diplomas, and proof of education in English letters through the DSBN Records Centre at the Education Centre.

The Consent to the Disclosure, Transmittal or Examination of School Record Form (to be used by student age 18 and over) or the Request for the Disclosure, Transmittal or Examination of School Record Form (to be used by parent of a student under age 18) CANNOT be used to order Transcripts or Diplomas.   1. Requests for an Ontario Student Record (OSR) from Current or Former Students or their Authorized Guardians

    To request your Ontario Student Record please complete and submit either a: Consent to the Disclosure, Transmittal or Examination of School Record Form (to be used by student age 18 and over) or a: Request for the Disclosure, Transmittal or Examination of School Record Form (to be used by parent of a student under age 18).

    Completed forms can be submitted to DSBN Legal Services by fax, postal mail, courier, or in person. DO NOT FORGET to include a copy of your ID along with your Consent to the Disclosure, Transmittal or Examination of School Record Form (to be used by student age 18 and over) or your Request for the Disclosure, Transmittal or Examination of School Record Form (to be used by parent of a student under age 18). Forms received without ID will not be processed until ID is submitted.

    The MFIPPA requires School Boards to charge a mandatory fee of $5 per student request plus a fee for photocopying and shipping.

    Note: If an OSR is being requested for more than one student, please complete and submit a separate form for each student.

  2. Requests for an Ontario Student Record (OSR) from Law Firms or other Third Parties

    Third parties (e.g. legal or medical professional) representing a client or patient, may also request an OSR upon written authorization from their client/patient.

    Third parties requiring an OSR for former students or students who are still enrolled in a DSBN elementary school, secondary school, or adult education centre, please submit a written request and either a signed authorization form from the student (age 18 and over) Consent to the Disclosure, Transmittal or Examination of School Record Form or the parent/guardian of a student (under age 18) Request for the Disclosure, Transmittal or Examination of School Record Form to:
