Criteria
- hockey players up to twenty years of age who have been a registered player in the Ontario Minor Hockey Association for a minimum of five years
- attending a post-secondary institution during the upcoming year
- maintain a high standard of academic excellence
- active in their local community programs
- athletic ability

- complete application form
- one-page letter outlining request
- one-page letter from the President of the local minor hockey association supporting their request
- proof of household family income
